You may continue to use the form previously developed. However, the Assessment Facilitators have developed a new form that is organized around each SLO (rather than by semester). We think you will like the new organization. You may move to the new form incrementally, if you wish. That is, you may want to complete the new form for just those SLOs that have assessment activity in the spring 2008 semester.
