Leave Requests
File a Leave Request Petition Form if you are planning to be out your first admitted term due to military obligations (in order to hold your admission) or if you are already matriculated and attending but will need to be out for two or more consecutive semesters (a Leave Request must be submitted before the start of your second semester out), with a maximum allowed of four semesters.
Before submitting this form, please review University Policy regarding Leave of Absence and Withdrawal by following these steps:
