This is a planning form to insure that the student’s MA plan will meet university requirements. | This should be used to help a student plan their MA course work. It is very helpful to have them submit a copy when they turn in their Candidacy form. There are two tabs at the bottom: one for the regular MA program and one for the Alternative Master’s Program. | ||
This is a planning form to insure that the student’s MS plan will meet university requirements. | This form applies to students entering our program PRIOR to the 2014/2015 academic year. It is very helpful to have them submit a copy when they turn in their Candidacy form. There are three tabs at the bottom: one for E&E, one for M &M and one for the SP. | ||
This is a planning form to insure that the student’s MA plan will meet university requirements. | This form has the same purpose as the aforementioned but applies to students enrolling in the program for 2014/2015. | ||

This is the petition for advancement to Graduate Candidacy. | This form is the graduate equivalent of the undergraduate major minor form. This form needs to be signed by the student, the advisor and the grad coordinator. | MUST be submitted prior to graduation. | |
This form replaces the old first page of the thesis that contains the signatures of the committee. | The form can be signed electronically. It testifies that the thesis has been approved by the committee. | This form needs to accompany the thesis when submitted. | |
This form is required for all grad students. It testifies that the student has completed their experience. | This form can be signed ONLY by the Graduate Coordinator. It is best if the faculty member initials the line that specifies when the degree requirements have been met. | This form is REQUIRED for graduation. | |
This is the graduation date request form for graduate students. | This form is REQUIRED for graduation. | ||
This is required if student have not completed their degree within two years of obtaining the RP grade. | The student must submit this if they do not want to have to reapply. | Must be filed before RP expires (2 yrs). | |
This form is required if a student wishes to change from one Masters program to another. | It is the equivalent of the undergraduate change of major form. |
