Graduate Course Frequency List [pdf] (effective Spring 2015)

     

Faculty Acceptance Online [pdf]

Used to accept graduate students into our program and can be signed electronically. This is the preferred acceptance form as it can be signed electronically. Required before the end of the semester in which the students apply.

Faculty Acceptance [doc]

Used to accept graduate students into our program. Hard copy only, cannot handle electronic signatures. Required before the end of the semester in which the students apply.

Transfer Course Validation Credit [pdf]

Allows student to transfer credit from another institution or from Open University. Up to 9 units can be transferred. MUST be submitted before the student applies to Candidacy.

Change Classification [pdf]

This will change a student’s status from conditional to classified.   MUST be submitted and accepted BEFORE the student applies for Candidacy.

MA Candidacy Worksheet

This is a planning form to insure that the student’s MA plan will meet university requirements. This should be used to help a student plan their MA course work. It is very helpful to have them submit a copy when they turn in their Candidacy form. There are two tabs at the bottom: one for the regular MA program and one for the Alternative Master’s Program.  

MS Candidacy Worksheet Pre-2014-15

This is a planning form to insure that the student’s MS plan will meet university requirements. This form applies to students entering our program PRIOR to the 2014/2015 academic year.  It is very helpful to have them submit a copy when they turn in their Candidacy form. There are three tabs at the bottom: one for E&E, one for M &M and one for the SP.  

MS Candidacy Worksheet 2014-15

This is a planning form to insure that the student’s MA plan will meet university requirements. This form has the same purpose as the aforementioned but applies to students enrolling in the program for 2014/2015.  

Graduate Advising Worksheet - Print [pdf]

This is the semester plan for a graduate student. This form is crucial for us to keep a record of active students. This is the hardcopy version. MUST be turned in EVERY semester.

Graduate Advising Worksheet - Online [pdf]

This is the semester plan for a graduate student. This form is crucial for us to keep a record of active students. This is the online version that can be signed electronically. MUST be turned in EVERY semester.

Candidacy [pdf]

This is the petition for advancement to Graduate Candidacy. This form is the graduate equivalent of the undergraduate major minor form. This form needs to be signed by the student, the advisor and the grad coordinator. MUST be submitted prior to graduation.

Thesis Committee Approval Form [pdf]

This form replaces the old first page of the thesis that contains the signatures of the committee. The form can be signed electronically.  It testifies that the thesis has been approved by the committee. This form needs to accompany the thesis when submitted.

Culminating Experience [pdf]

This form is required for all grad students. It testifies that the student has completed their experience. This form can be signed ONLY by the Graduate Coordinator. It is best if the faculty member initials the line that specifies when the degree requirements have been met. This form is REQUIRED for graduation.

Award Degree [pdf]

This is the graduation date request form for graduate students.   This form is REQUIRED for graduation.

RP Extension [pdf]

This is required if student have not completed their degree within two years of obtaining the RP grade. The student must submit this if they do not want to have to reapply. Must be filed before RP expires (2 yrs).

Change Major [pdf]

This form is required if a student wishes to change from one Masters program to another. It is the equivalent of the undergraduate change of major form.
